The Ontario Health Study has a partnership with LifeLabs to collect blood samples for the Study. LifeLabs is a Canadian company that handles more than 100 million laboratory test annually. The have clinics in communities across the province.

Requisition Form

After you complete the blood collection questionnaire and consent, your requisition form will be available for you to download and print. The OHS requisition form identifies OHS participants to LifeLabs® Medical Laboratory Services and informs the staff that the blood sample will be taken according to Study procedures.

If you cannot print the requisition form or you do not have access to a printer, you will need to take note of the Participant LID number and verification code displayed on the top left portion of your requisition form. LifeLabs® Medical Laboratory Services will be able to print a blank requisition form at the patient centre when you arrive for your appointment and fill in your information.

Finding a LifeLabs and booking an appointment

LifeLabs is in the process of training employees at all of their locations across Ontario to accept samples for the Ontario Health Study. Until this process is complete, please ensure you visit a participating location to provide your OHS blood sample.

When using the LifeLabs website to find your nearest location, enter your postal code into the Location Finder search bar, select the ‘Ontario Health Study’ checkbox and then click ‘FIND’. Please see below:

All LifeLabs locations will be ready to accept samples for the Study in the coming months. There is no deadline for proving a blood sample to the OHS so if the only conveniently-located lab near you is not yet a participating location, you will be able to take part once the transition is complete.
