The Ontario Health Study collects health data by asking participants to fill out an online questionnaire about their health. The OHS follows the health of participants by periodically conducting follow-up questionnaires which gives participants a chance to provide an update on their health.
As part of your registration for the Study, you will be asked to fill out our baseline questionnaire. It takes approximately 45 minutes to an hour to complete and looks at things such as:
- Personal and family health history
- Sleep patterns
- Physical activity
- Emotional health and well-being
- Alcohol and tobacco use
- Environmental exposures

Because the Ontario Health Study is a long-term study, one of the things that researchers are interested in how your health changes over time. We track our participants’ health over time by asking them to fill out follow-up questionnaires from time to time.
A number of questions are similar to those on the baseline questionnaire. The questions repeated are those that you might answer differently such as your weight and habits such as alcohol and tobacco use.
The OHS will be conducting its first follow-up questionnaire. In addition to collecting updated information about your health, lifestyle, medical and family history, there are new questions related to:
- eCigarette use
- Marijuana use
- Over-the-counter medication use
- Mental health
